New Patient Form

Prior to your first appointment, please take the time to fill out our New Patient Form.

New Patient Form

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

What to Expect

When you come in for your first visit, we will ask you to fill out a New Patient Form and a Medical Form. We will complete a full oral exam in order to help determine your dental needs and review your past medical and dental history.

  • Review your medical history and dental history, including any previous dental work
  • Perform a complete oral exam
  • Create a comprehensive and personalized dental health plan

Insurance Policies

We recognize most dental insurance policies. When you come for your appointment, please bring your insurance information. On your behalf, we will submit your treatment details to your insurance company.

Since there are many different insurance plans, we have no definitive knowledge of any given plan and urge you to know your own coverage. Should you have questions, please call your insurance provider.

Financial Policy

Fees are due and payable on the day of service.  Payments can be made using the following: cash, debit, VISA, MasterCard, and personal cheques.

Cancellation Policy

In the event of a missed appointment with less than 48 hours notice, a deposit will be required to reschedule another appointment. The deposit will be applied to you next scheduled treatment fee. However, if this second appointment is missed, the deposit will be forfeited.

Tips From Our Team:

  • We highly recommend limiting the amount of sugary food you consume because the bacteria in dental plaque can change sugars into acids causing damage to your teeth.
